If you are looking for aged care for yourself or a loved once, Care Guidance will assist you find an appropriate aged care facility, and there will be no cost to you.

Care Guidance works with a wide range of aged care providers (their names are listed on our website) and our partners have agreed to pay our fee should you decide to choose one of their homes.  Our partnership network covers approximately 80% of aged care beds across Victoria.

It is true we do not have a partnership agreement with every aged care home in Victoria, but our partners’ homes have the breadth and depth of care options that most of our clients require.

By “breadth and depth” we mean the breadth of geographical location of homes across Victoria, so that it is likely there is a home option (or multiple home options) in the area you are looking and also the depth of homes to cater to the care needs – from low care through to complex and memory support needs.

But, if  you wish us to look for a specific home – inside or outside our partnership network – we will do that and provide the same level of service, the only difference being in one instance we will be paid and in the other, we won’t.

The fee we charge our partners is a flat, one off, fee on you (or your loved one’s) successful transition into the chosen aged care facility.  There is no on-going fee, there is no fee for next year and there is no fee based on any % of your cost of aged care.  It is a one-off standard across our providers.

There is no reason why you can’t undertake the research and work to find a suitable aged care home, many people do.  However, for many others the shock and unexpectedness of the decline in loved one’s health situation, the complicated nature of aged care rules, the time that needs to be dedicated to the task, the feeling of helplessness about their loved one and the lack of knowledge about what and how to do it renders them unable to undertake the task themselves.  Sometimes, even family dynamics add to already difficult circumstances.  And there could be liaisons with hospital and social workers to undertake.

We alleviate as much stress in the process by acting on your behalf and liaising with the right people.  Making the transition into aged care a better experience for everyone.

In learning about us, you may have been given our name by a social worker at a hospital of the hospital may have told us about you, or you may have been referred by the organisation that is involved with  your loved one’s home care package or you might have been told about us by a friend, colleague or neighbour.  Whichever way you come to us, we will need some initial information to get the ball-rolling.  This is where one of our Aged Care Specialists (i.e. the person you will work with during this process) will contact you and ask:

• Your loved one’s name and health condition
• Where they are currently (home, hospital etc.)
• Who the next of kin is – i.e. who is going to make decisions and is there anyone else involved (partner, sibling, friend)
• The suburb that you are looking in.
• Do you think the placement is going to be a permanent relocation or a short-term relocation?
• Your contact details.
• If in hospital, who are you dealing with?
• Have you already contacted some aged care homes, if so, which ones?  If not, please do not do so, that is the skill and expertise we bring.  We know who to speak to and we know homes.  For example, the home “just down the road” may not be suitable for your loved ones care need, or it may not have a vacancy.
