Situations We Help With

You do not need a diagnosis or a formal referral to get in touch. If any of these feel close to home, the Wellbeing Program may be exactly what your family needs.

"
"Dad barely leaves his room anymore"

He has become increasingly withdrawn, less engaged, less connected and harder to reach. Isolation in Aged Care is more common than most people realise, and it has real consequences for health and wellbeing.

"
"Mum hasn't seemed like herself since she moved in"

Adjusting to aged care, or to a significant change in health, can be quietly devastating. When someone is struggling to adapt, specialist support makes a real difference.

"
"Dad's been through so much. I think it's catching up with him"

Trauma does not disappear with age. For many older Australians, past experiences resurface during periods of change, loss or vulnerability, and they deserve to be taken seriously.

"
"Mum's condition is getting more complex and we don't know what to do"

Complex care needs, whether physical, cognitive or emotional, require more than clinical management. Our social workers work alongside care teams to address the full picture.

"
"We don't know how to support Dad through this diagnosis"

A new diagnosis changes everything. Families often feel underprepared and unsupported at exactly the moment they need help most.

"
"We cannot agree on what is best for Mum"

Care decisions can surface deep family tensions, especially around finances, legal matters, or end-of-life planning. Our social workers help families find clarity and move forward together.

Support For You, Too

Caring for a loved one through aged care can take a quiet toll on a family. A Wellbeing Specialist can also walk beside you and your family through this time, available privately as a fee for service. Get in touch to find out more →

How It Is Funded

Accessing the Wellbeing Program

The Wellbeing Program can be accessed in several ways, whether your loved one is living at home with care support or in a residential aged care setting.

Support at Home
Funded Through a Support at Home Package

If your loved one has a Support at Home Package, the Wellbeing Program can be funded as an allied health service within that package. Your care manager or Care Guidance can help explore this option.

Residential Aged Care
Funded Through a Residential Aged Care Provider

The Wellbeing Program is embedded directly into residential Aged Care facilities through a partnership with the provider. If your loved one's home does not yet offer the program, Care Guidance can help facilitate a conversation with the provider, or you can contact us directly.

Private
Funded Privately

If your loved one is not in an Aged Care home and does not have a Support at Home Package, Care Guidance can explore how the Wellbeing Program could be funded privately. Get in touch and we'll talk through the options with you.
